An exciting opportunity is available for experienced and innovative health care professionals with an interest in respiratory to be part of a team that will establish a Rapid Respiratory Response (RRR) service. These post will be of interest to both Registered Nurses and Physiotherapists.
The RRR service is a new and exciting service development which has been tested as a feasible and effective service. We are now expanding across the three HSCP and wider respiratory services with developed pathways with AUCS, CAU and SAS. The aim of the Rapid Response Respiratory Team service is to provide timely, and appropriate respiratory specialist input to enable patients experiencing a COPD exacerbation to remain at home whenever possible.
We provide individualised and clinically effective care, including screening, assessment, treatment to maximise patient health and wellbeing, and promote self-management. The service provides symptom management, rehabilitation, education and training to adults with a confirmed respiratory diagnosis.
The service is currently available from 9am until 5pm Monday-Friday, with a view to extending to the weekends.
Some key dimensions/aims include:
• To improve patient outcomes and experience
• To avoid COPD-related attendances at the Emergency Department
• To avoid GP referrals to Combined Assessment Unit – admissions to hospital
• To facilitate early discharge from hospital
• To reduce COPD readmissions by improving patient knowledge of condition, and developing skills and confidence to self-manage. Current COPD readmission rate within 28 days is significant.
This is a fixed term/secondment opportunity until March 2024.
Full driving licence and car owner essential.
Hours: 37.5 per week (Part time or job share will be considered).
